2012 Registration of Legally Blind and Visually Impaired Students

Attn: Directors of Special Education and Teachers of the Visually Impaired

This is the official request for the registration of legally blind students under the Federal Act, "To Promote the Education of the Blind," and for the registration of visually impaired students under the Kentucky State Textbook Program.

  • Students who have been registered in the past must have current eye reports (not older than January, 2009). Students not previously registered need to have a current eye report (not older than January, 2011). Please check your records for the status of students to be enrolled.

  • If you do not have a current eye report for students who are legally blind or visually impaired, we still want you to list their name and the other pertinent information on the “Certificate of Enrollment“. When you obtain the eye report, please forward it to KIMRC so your student can receive materials from us.

You will need the following materials to complete the Federal Quota Registration. They can be accessed by clicking on the link.

  1. Definitions of "Legally Blind," and "Visually Impaired" and Explanation of Abbreviations

  2. Certificate of Enrollment Form (An indication of the student's previous local school program if he or she has just moved into your program; or the location of the student's current education program if he or she has moved from your program would be helpful on the Certificate of Enrollment.)

  3. Assurance of Compliance with the Department of Health, Education, and Welfare Regulation under Title VI of the Civil Rights Act of 1964

  4. Registration and Eye Report Form for Students with Visual Problems

Please note that there are two types of conditions for registration. Legally blind students are registered under the Federal Quota Registration (FQR). Visually impaired students are registered under the State Textbook Program.

Students must be enrolled in an early intervention program or grades preschool through programs of less than college level and must be enrolled in your school district or with your agency as of the first Monday in January of the current school year (i.e., 2011-12).

Students whose eye reports are outdated are required to have a current eye report on file. Students who are registered for the first time also need a current eye report. An exemption from the current eye report regulation will be made for persons who are totally blind or whose eyes have been enucleated or who have proven non-changing eye conditions. If this is the case, we must receive a letter from an ophthalmologist or optometrist stating the non-changing eye condition.

The following items must be included on the eye report form:

  1. Complete the information regarding name, address, school district, date of birth, etc.

  2. The current grade level of the student is required.

  3. How Served: IEP VI/Only, IEP VI/Multiple, 504 Plan, or Other.

  4. Primary Reading Medium*: Print, Braille, Auditory, Prereader, Nonreader

  5. Secondary Reading Medium*: Print, Braille , Auditory, Not Applicable

  6. Have the examining ophthalmologist/optometrist or his office staff complete as much information as possible within sections I, II, III, and IV.

  7. The eye specialist, the school Superintendent or Director of Special Education and the Parent/Guardian must sign and date the eye report form.

* Do not duplicate the primary and secondary reading medium.

The “Certificate of Enrollment,” “Eye Report for Students with Visual Problems,” and “Assurance of Compliance” must be completed and returned to this office on or before February 3, 2012 .

  • IF THE FORMS ARE NOT RECEIVED BY FEBRUARY 3, 2012, IT WILL NOT BE POSSIBLE FOR THE KIMRC TO REGISTER YOUR STUDENTS TO RECEIVE MATERIALS UNDER THE FEDERAL ACT DURING THE 2012-2013 SCHOOL YEAR. These students must be enrolled in your school(s) as of Monday, January 2, 2012.

  • If there are no students who are legally blind or visually impaired enrolled in your school(s), please return the “Certificate of Enrollment” marked "NONE", signed, and dated.

Last year, the Federal Quota Allotment was $348.61 per student. This year's quota amount has not been set. However, it is important to register every legally blind student that is enrolled whether they are a 12 th grade student or a nine-month-old infant (e.g., VIPS, First Steps).

Please print and complete the “Certificate of Enrollment” and “Assurance of Compliance” forms.

  • CHECK ONLY ONE BOX in the “How Served” column for each student on the Certificate of Enrollment. As you know, many students who are blind or visually impaired are served through IDEA in varying categories. If the student is being served through an IEP, check “VI Only” if no other disability has been identified, OR check “VI Multiple” if the student has a visual impairment in addition to any other type or combination of disability (e.g., VI+FMD, VI+LD, DB, VI+ MMD, VI+DD, etc.). Some students are provided adaptations/modifications through a 504 Plan, if so, please check that box. For students who are home-schooled, are served in private or other type of school settings or receive large print textbooks, or not, without accommodations being documented in any way, please check “Other.”

  • It is also very important that you indicate both the Primary Reading Medium (V=Visual (Print), B=Braille, A=Auditory, N=Nonreader, P=Prereader) and the Secondary Reading Medium (V=Visual (Print), B=Braille, A=Auditory, NA=Not Applicable) on the Certificate of Enrollment. Please remember that you may not duplicate a reading medium in the primary and secondary reading media columns.

  • You must indicate the Primary Language of the Learner. This should be done in the Student & Language Column. The language categories are:

  • English (E)

  • Spanish (S)

  • Other (languages other than English and Spanish submitted to you when you gather your data: e.g. Spanish, ESL, etc.) (O)

  • Unknown (U)

  • N/A (not applicable-for those registrants who do not use speech due to age or developmental level: e.g. infants, etc.)

  • Enter E, S, O or U next to the Students Name

  • When the forms are completed with the proper information and signatures, please return them to the KIMRC

As you know, this process determines funding allocations to provide braille and large print textbooks as well as specialized educational aids.
